Volunteer Application

This form comes directly from Little League Baseball. After coaches are selected they will be expected to fill out this form, provide a photocopy of their driver's license and submit to the following address:

Medical Release

Coaches should have this form on hand for each player signed by the parent or guardian. It may be required to seek medical treatment in the absence of the parent/guardian.

Coach Orientation

Coaches will be oriented in Western Hills Baseball policies during their draft or preliminary meetings. All coaches must attend a pre-season coaching clinic.  Date TBA.
